Dr. Arita wants to answer as many of your questions as possible at your visit.

Please prepare for this by bringing:

 

• Photo ID 

 

• Insurance card

 

• Radiology studies (actual films or CD, and reports) pertaining to your current medical problem (including X-rays, MRI, CT Scan, CT Myelogram, etc.)

 

• The names, phone and fax numbers, and addresses of all physicians involved in your care (Please be sure to include primary care physician)

 

• Prior medical records from another pain physician, surgeon, rheumatologist, neurologist, or anyone involved in your care (Please let us know if you were Dr. Arita's patient at UCLA as we have to obtain medical record)

• Completed New Patient Questionnaire/Insurance Information (for all first-time visits)
